In a first of its kind collaboration that transforms how primary care is delivered to its members, Prominence Health Plan is partnering with Vera Whole Health to integrate care delivery into Medicare Advantage and select employer group health plans. Launching in January 2019, the new Prominence Care Centers powered by Vera will help meet the unprecedented demand for accessible and high-quality health care from Prominence Health Plan in Reno and Carson City, Nevada.
Vera helps patients achieve their ultimate well-being, while coordinating and managing healthcare costs. Full-time medical teams and certified health coaches can meet most care needs through preventive care, chronic disease management, acute care and integrated health coaching. The care centers also offer on-site labs and provider-dispensed medications at no cost.

Prominence Medicare Advantage members without a primary care provider (PCP) can select a Vera provider at the care centers as their PCP. Members can schedule wellness exams and screenings or use the care centers for acute care and chronic disease management. The medical team at each care center can also serve as an extension of care to members who have an established PCP.
Medicare beneficiaries can enroll in Prominence Health Plan Medicare Advantage plans until the end of open enrollment on Dec. 7, 2018.
